By signing below, I voluntarily agree to participate in the Glo Karts Chicago electric golf cart tour experience. I acknowledge and assume all risks associated with participation, including but not limited to personal injury, property damage, and accidents.
I agree to follow all safety instructions provided by Glo Karts Chicago staff at all times during the experience.
I hereby release and discharge Glo Karts Chicago, LLC, its owners, employees, agents and representatives from any and all claims, demands, damages, causes of action, and liabilities arising from or related to my participation.
I confirm that I am 18 years of age or older, or, if a minor, that a parent or legal guardian has completed this waiver on my behalf.
I understand that failure to comply with safety guidelines may result in immediate removal from the experience without refund.
